The HL7 FHIR at Scale Taskforce (FAST) FHIR Accelerator continues to gain momentum as it advances critical infrastructure for scalable, secure, and trusted FHIR-based interoperability. With the release of the new HL7 Foundry FAST Bundle, the community now has access to a powerful set of resources that integrate FAST Security and FAST Identity capabilitiesāessential building blocks for a connected healthcare ecosystem.
FAST has long focused on solving foundational challenges like scalable security, digital identity, and consent management to support a future of nationwide, API-driven data exchange. Its work is critical to ensure that as healthcare organizations adopt FHIR, we do so in a way that is consistent, secure and aligned with regulatory expectations.
Key projects in 2025:
FAST Security IG
Scalable, dynamic registration, authentication and authorization for FHIR ecosystem participants, leveraging trust frameworks, UDAP, OAuth and PKI-based credentials. Moving toward STU2 with enhancements to community support and scope negotiation
FAST Identity IG
Best practices for patient matching and digital identity management across payer-provider workflows, supporting both B2B and B2C interactions. A new version is coming soon with enhancements throughout
FAST Consent IG

A federated approach to consent tracking across multiple management systems, allowing patients to easily update or revoke consent across the network
FAST + Da Vinci: Powering Value-Based Care and Automation
FAST and the Da Vinci Project are now working closely to integrate these foundational capabilities into high impact use cases. While FAST focuses on creating the infrastructure for scalable FHIR exchange, Da Vinci accelerates adoption through real-world workflows in value-based care, such as:
- Prior authorization
- Payer data exchange
- Coverage discovery
- Patient cost transparency
Da Vinciās Project Trebuchet is actively testing FAST solutions to ensure they can meet the demands of large-scale, multi-stakeholder environments. Together, they are helping transform todayās point-to-point integrations into a true interoperability ecosystemāsimilar to what X12 has done for administrative transactions and NCPDP for pharmacy.
